Anti-Coagulation Policy

It is a practice requirement within NHS England Prescribing Department Agreement, that every patient on anti-coagulation (Warfarin), must produce their paper letter at the time of the request, as the practice must be sure that the correct dosage has been asked for, and this can only be confirmed by entries in that document. Without the letter you will only get 7 tablets. This is for your own benefit and safety.
